Vaccination Verification Instructions

All participants at the in-person 2021 ASHA Convention in Washington, D.C., whether a presenter, attendee, exhibitor, staff, guest, or vendor, will be required to be fully vaccinated. You must provide proof of vaccination to register for the in-person event.

ASHA is using a HIPAA-compliant service called CrowdPass to verify vaccinations. The online process is simple, secure and should only take you a couple of minutes. We strongly recommend that you complete your verification the day before you plan to register. CrowdPass follows a thorough two-step verification process which may take up to 12 hours during busy times. Most verifications are completed within an hour or two, but please be patient if you do not receive your confirmation until the next day. We do not have a cap on registration and you do not need to be registered to reserve a hotel room.

Your vaccination verification code is unique to you and may not be shared or reused.


Vaccination Verification Form:

You will need your vaccination card on hand (or a copy/photo of it saved on your computer) so you can reference the information on it and then take or upload a picture. Accepted file formats for your upload are .png, .jpg, .jpeg, or .pdf.

  1. To begin, you will log into CrowdPass with your Google or Facebook credentials, or you can create an account with an email address. (You may receive an email asking you to confirm your email address. Click confirm to proceed with the verification process.)
  2. Provide some brief contact information using your name exactly as it appears on your vaccination card, agree to the CrowdPass waiver, then answer yes to the question that you are fully vaccinated.
  3. Fill in the information from your vaccination card, indicating which vaccine you received and when you received each dose. (If you received your vaccination outside of the United States, select Other. Crowdpass will verify international vaccine cards, but it may take up to 12 hours to receive your email with a verification code. Non-standard U.S. vaccination cards may also take additional time to be approved.)
  4. Take or upload a photo of your vaccination card and hit Submit.
  5. You will receive a confirmation email from CrowdPass that has your vaccination verification code that you will enter into the ASHA Convention registration system. Please be patient if you do not receive your confirmation until the next day. Most people receive their confirmation email within a couple hours, however, it may take up to 12 hours during busy times. Be sure to check your junk or spam folder if you do not receive this message.

Tip: If you have uploaded your vaccination card and would like to check your approval status, or if you lost your confirmation email, you can log back in through the link above using the same email. If you have been approved, you can click the green approval icon to view your verification number.

If you are unable to use the CrowdPass verification system, please e-mail so we can schedule an alternate verification process.

Technical Assistance

Please contact if you experience any technical issues with the verification process, if it has been more than 12 hours since you uploaded your vaccination information but have not received a confirmation and verification code, or have an unusual situation (such as a non-standard vaccination card) and need assistance.


Accommodations may be offered on a case-by-case basis for any in-person participant who is unable to be vaccinated due to disability or sincerely-held religious beliefs. E-mail to request the form.
