2008 Open House Reservation Request
Please reserve space for us to hold an Open House during the 2008 Annual Convention in Chicago, Friday, November 21, from 7:30 pm - 9:30 pm. We understand that when we are notified of our space assignment, we will be responsible for completing all catering arrangements and handling all charges for food and beverage.

Note: This year's Open Houses will be held at the Chicago Hilton Hotel. The hotel has worked with ASHA and has developed a few food and beverage packages for you to choose from that are reasonably priced. If you desire, you may still work with the hotel to create your own customized food and beverage menu.

Space will be assigned on a first come basis.
